You'll need to read and agree to this contract before you book your telephone consultation with Fred Amir.

Authorization:

The client is engaging Fred Amir, author of Rapid Recovery from Back and Neck Pain, to provide telephone consulting about matters of interest to the client.

Notification:

A 24-hour notice is required for rescheduling or cancelling an appointment. Otherwise client is charged for a full session. In rare cases when due to an emergency client is not able to provide this notification the fee can be applied to a future session.

Overage:

Each session is 50 minutes or 25 minutes long. Additional time is charged in 10 minute increments at $25 each.

Legal Stuff:

Health Advisory Group and Fred Amir do not warrant that the advice given in this consultation will meet the client's requirements or be free of errors and omissions. Client understands that Fred Amir is not a medical doctor and he does not provide medical advice. Client must seek medical attention for his/her medical condition. Client also understands that effectives of the Rapid Recovery plan vary with each individual. In no event will Health Advisory Group or Fred Amir be liable to the client or any third party for any damages or losses, or other incidental, consequential or special damages arising out of information provided. Since to be effective, plans must be executed with knowledge and experience, and the client may not possess that knowledge and experience after the consultation, or may misinterpret or misunderstand advice given, the entire risk as to the results of the consultation is with the client.

Severability:

If any provision of this agreement shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable from this agreement and shall not affect the validity and enforceability of any remaining provisions.

Venue:

Regardless where the client resides, the client agrees that for purposes of venue, this contract was entered into in Santa Clara County, California, and any dispute will be arbitrated in Santa Clara County, California.

Sole Agreement:

The agreement contained in this "Telephone Consulting Contract" constitutes the sole agreement between Health Advisory Group and the client. All prices specified in this contract are stated on the website and are subject to change without notice.

Refund Policy:

No refunds are given for telephone consultation.
